Prior to your first appointment, you will be required to complete paperwork that reviews my office policy, HIPAA policy, and a document that asks for information
about your personal history and background. If you would like to use your insurance benefits, complete that form as well. Please bring these with you to your first appointment.
Use your first appointment with me as an opportunity to ask questions about my practice and approach to therapy. Counselors are not all alike and each modality is
not a “one size fits all” approach. Just as some people work well with and can relate to some teachers and others not so much, it is the same way with counselors and other
service providers. We will decide together if my services can be a help to you and if it feels like a good fit.
Please complete the following forms prior to your first session and mail them to my office. If you plan on using health insurance, verification of benefits prior to your first visit is required. Initial paperwork forms are available below in PDF format.
Please review my policy for reviewing and obtaining your mental health records. According to WAC 246-809-035 records will be retained for 5 years after the termination of treatment.
